Our forms are on our patient portal, not on this website. Fill them in before you come, or arrive 15 minutes early and we will help.
What happens

Step 1: Book
Use the booking link or call. Tell us if you need an interpreter or a longer visit.

Step 2: Forms
Fill in the patient forms ahead of time, or at the office.

Step 3: The visit
An exam or a myopia evaluation, with the doctor explaining each finding.

Step 4: Before you leave
A copy of your prescription, a written estimate for anything you pay yourself and the next visit, if there is one.
